A senior care provider is opening a new “clinical hub” for part of the region.

ConcertoHealth said this month it will open a 14,500-square-foot clinical and administrative office on June 1 in Kalamazoo, at 6563 W. Main St.

The organization said it has invested $400,000 in the new facility, which will serve as its central location for the southwest Michigan market.

The Kalamazoo office will provide telephonic and field-based care, as well as offer network support to partner primary care physicians.

"This will be a collaborative space where primary and specialty care clinical professionals and community-based behavioral health and social workers can meet with patients and their caregivers," said Bobby Lee, medical director, ConcertoHealth Michigan.

Previously, ConcertoHealth had been operating from a temporary space.

The new office will accommodate 90 positions and allows the organization to expand its footprint in the state, which also includes four care centers in greater Detroit.

Irvine, California-based ConcertoHealth provides individualized care for frail and elderly patients via an on-the-ground clinical staff to extend the reach of primary care physicians.

The organization delivers patient support via a care team comprising nurse care managers, care coordinators, social workers, pharmacists, home-visiting doctors and dietitians.
